Founding Cases

You were told your labs are normal.

Twelve to fifteen sponsored places. Six months of concierge care for the conditions generic medicine dismisses, funded by Uplevel and documented properly. You bring the case; we do the work.

An open case file on a desk: a leather document case, a kraft folder of charted lab results, blood collection tubes, glass vials, and a fountain pen.

The cohort
  • 01

    Endometriosis

Years to a diagnosis, then care that stops at the prescription. We work the systems around the disease: inflammation, hormones, sleep, recovery.

A sealed amber glass ampoule lying on a limestone block under warm directional light.
  • 02

    Autoimmune thyroid

Hashimoto's and Graves'. A TSH-only workup calls you normal while antibodies, conversion, and symptoms say otherwise. We run the full panel and read it as one system.

    2 to 3 places
A frosted glass butterfly, the shape of a thyroid, resting on a limestone block.
  • 03

    Midlife hormonal transition

Perimenopause on one side; a testosterone slide dismissed as 'just aging' on the other. Same window, same shrug from generic care. We work up the drivers first — sleep, stress, metabolic — and read hormones, bone, and body composition as one system.

    2 to 3 places
A glass hourglass with golden sand mid-flow, standing on a limestone block.
  • 04

    Cardiometabolic prevention

A standard panel calls your cholesterol fine and stops. ApoB, Lp(a), and insulin resistance tell the real story. We measure what predicts risk, then work the numbers that can move.

    2 to 3 places
An anatomical heart of frosted glass glowing softly on a limestone block.
  • 05

    Complex hormonal + metabolic

The 'everything looks fine, but I feel terrible' cases. Multi-system symptoms, normal-range labs, no one owning the whole picture. That picture is the product.

What Uplevel funds
  • Six months of full membership, funded by Uplevel
  • Complete mechanistic panels at months 0, 3, and 6
  • A licensed clinician who owns your whole picture
  • Care that runs alongside your existing specialists, never replacing them
  • Wearable integration and the responsive care team
What we ask
  • Real engagement: monthly check-ins, the re-testing cadence
  • Validated symptom measures at baseline, 3, and 6 months
  • Your story at baseline and at the end, in your words
  • A refundable $399 deposit at acceptance, returned in full when you complete the study
  • Layered documentation consent, signed after selection
  • Going public with your name is optional and never a condition

Documented properly, or not at all.

Every case rests on three things: objective markers (the full mechanistic panel, including what a standard workup skips), validated symptom instruments scored the same way at months 0, 3, and 6, and your narrative, captured before anything begins.

Clinicians document what actually happens, including partial and null results. Nothing is published without your review and approval, de-identified if you prefer, and any public story is disclosed as a sponsored founding case. This is investigation and optimization alongside your existing specialists. It is not a cure, and we will never claim one.

Selection runs both ways.

01

Apply

A short application: your condition, your history, your story. No labs, no fees.

02

Screening call

Shortlisted applicants talk with the team. Fit runs both ways.

03

Clinical intake

A clinician reviews your history and confirms the program can genuinely help your case.

04

Offer

Selected cases place a refundable $399 deposit, sign the layered consent, and begin. The deposit comes back in full at completion.

Same clinical standards as every member · Available in 47 states · Not for acute or unstable presentations

Apply for a founding case

Founding cases receive fully funded care in exchange for documented participation and a refundable $399 deposit, returned in full at completion of the study. Any published story is disclosed as sponsored, is approved by you first, and reflects one person's documented experience, not a typical or guaranteed result.
